Penis Filler Risks: What a Fatal Hyaluronic Acid Case Reveals About “Non-Surgical” Penis Enlargement

A recent death following hyaluronic acid penis filler is raising an important question for men considering penis enlargement: does “non-surgical” actually mean safer?

Penis filler, penile filler, fat injections and other so-called “non-surgical penis enlargement” procedures are often marketed using reassuring terms such as natural, minimally invasive, temporary, reversible and non-surgical.

Those words may describe certain characteristics of a treatment.

They do not, by themselves, establish that the treatment is safer than penis enlargement surgery.

The recent death of British-Nigerian businessman and influencer Igho “Tiny” Ubiribo following a hyaluronic acid penile filler procedure provides a sobering reminder of that distinction.

Quick Answer: Are Penis Fillers Safer Than Penis Enlargement Surgery?

They should not automatically be assumed to be safe simply because they do not involve surgery.

Serious complications from penis enlargement injections have been published in medical literature such as fatal pulmonary embolism following hyaluronic acid penile filler and fatal fat embolism following autologous fat injection for penis enlargement.

What Happened After Igho Ubiribo Received Penis Filler?

In March 2026, Igho “Tiny” Ubiribo underwent a cosmetic penis enlargement procedure at a clinic in Bangkok, Thailand.

According to reporting on evidence from the subsequent UK coroner's inquest, approximately 40 mL of hyaluronic acid filler with lidocaine was injected during the procedure.

Ubiribo developed chest pain afterward, collapsed and was transported to a hospital. Despite prolonged resuscitation efforts, he died early the following morning.

The coroner concluded that his death resulted from a pulmonary embolism associated with the hyaluronic acid injection. Thai health authorities subsequently opened an investigation into the clinic and circumstances surrounding the procedure.

This was not simply swelling, bruising, an uneven cosmetic result or filler migration.

It was a fatal systemic complication following a cosmetic penile filler procedure.

Can Hyaluronic Acid Penis Filler Reach the Lungs?

Yes. Although rare, medical literature shows that injected filler material can enter the circulation and reach pulmonary blood vessels.

This is known as a non-thrombotic pulmonary embolism because the material obstructing blood vessels is not necessarily a conventional blood clot.

And the Ubiribo case is not the only documented example involving penis enlargement.

A forensic case report published in 2025 described a 56-year-old man who underwent penis enlargement using a hyaluronic-acid-based injectable material.

He developed shortness of breath and systemic symptoms on the day of treatment and died the following morning.

During autopsy, investigators found gel-like foreign material not only around the penis but inside numerous blood vessels in the lungs. Testing identified the material as hyaluronic acid or a hyaluronic-acid salt.

A separate review of filler-induced non-thrombotic pulmonary embolism following genital aesthetic injections identified cases involving hyaluronic acid, autologous fat and other injected substances. Some were fatal.

While rare, this is a serious and extreme risk for a cosmetic penis enlargement procedure.

“Non-Surgical” Is Not a Medical Definition of Safety

This is where patients need to be especially careful with penis enlargement marketing.

Search for penis filler, penile filler, penis girth enhancement or non-surgical penis enlargement, and treatments are frequently presented using phrases such as:

• non-surgical;
• minimally invasive;
• natural;
• biocompatible;
• reversible;
• little downtime; and
• no general anesthesia.

Some providers go further and characterize penis filler as a lower-risk alternative to penis enlargement surgery.

But the absence of an incision does not eliminate medical risk.

An injection still introduces material into living tissue containing blood vessels, lymphatics, nerves and other anatomical structures.

In the case of a filler embolism, the very fact that material is injected is central to the mechanism of the complication.

That is an important distinction when comparing penis filler with a pre-formed penile implant.

“Natural” Does Not Mean Risk-Free Either

Hyaluronic acid is naturally present in the human body.

Autologous fat comes from the patient's own body.

Both facts can sound reassuring.

Neither means that injecting those substances into the penis is automatically without serious risk.

In fact, autologous fat—the patient's own natural tissue—has also been associated with a documented fatal penis enlargement complication.

A peer-reviewed 2017 case report described the sudden death of a healthy young man following penis enlargement using autologous fat transfer.

The investigators concluded that fat injected into the penile shaft entered the circulation and caused a fatal fat embolism.

This is an important lesson for anyone evaluating male enhancement procedures:

“Natural” describes where a material comes from. It does not prove that injecting that material into a particular anatomical location is safer.

Are Hyaluronic Acid Penis Fillers FDA Approved?

Dermal fillers may be FDA-approved products, but that does not mean they are FDA-approved for penis use or penis enlargement.

This distinction is frequently misunderstood.

FDA-approved dermal fillers have been evaluated for specific indications involving areas such as the face and hands.

The FDA's published approved indications do not include cosmetic penile enlargement.

Therefore, a clinic saying it uses an “FDA-approved filler” does not mean FDA has approved that filler for injection into the penis.

Those are two very different statements.

Using an FDA-approved product for a different anatomical location or indication may constitute off-label medical use. Off-label prescribing and use can be part of legitimate medical practice, but patients should understand that FDA has not evaluated that particular product for safety and effectiveness for every off-label use.

For a man considering dermal filler penis enlargement, a reasonable question is therefore:

“Is this product FDA-approved specifically for injection into the penis?”

That is much more meaningful than simply asking whether the filler itself has FDA approval somewhere else in the body.

Penis Filler vs. Himplant Penis Enlargement Surgery: They Are Not the Same Thing

One of the biggest sources of confusion in the penis enlargement market is treating every enhancement procedure as though the only meaningful difference is whether it requires surgery.

It is not.

The last distinction deserves particular attention.

The fatal complications described with hyaluronic acid and fat involve injected material entering the circulation.

Himplant does not enlarge the penis by injecting free filler material.

It is a pre-formed implant.

How Is Himplant Different From Penis Filler?

FDA's 510(k) record for the Pre-Formed Penile Silicone Block states that the device is intended for use in augmentation, reconstructive and cosmetic surgery and, when used for augmentation, provides cosmetic augmentation of the penis for aesthetic purposes.

The FDA submission included clinical data from patients who underwent cosmetic penile surgery with the device.

That is an entirely different regulatory situation from taking a dermal filler approved for another anatomical indication and using it off-label for penile enlargement.

Have Deaths Been Reported in Published Himplant Clinical Studies?

No deaths were reported in the published Himplant/Penuma clinical studies reviewed for this article.

Published Himplant clinical research includes a large retrospective clinical study involving 400 consenting patients from a 526-patient surgical series, a separate tertiary-care-center experience involving 49 patients, a 92-case study evaluating surgical approaches and patient satisfaction, and additional Himplant clinical research.

These studies reported outcomes and monitored adverse events, but mortality was not reported in these study populations.

No medical procedure should ever be described as having zero risk.

But patients should also not be encouraged to believe that choosing an injection instead of penis enlargement surgery automatically removes the possibility of a severe complication.

The evidence does not support that assumption.

“Reversible” Does Not Mean Every Complication Is Reversible

Another frequent penis filler marketing message is that hyaluronic acid is reversible because it can often be dissolved using hyaluronidase.

That can be a legitimate advantage of HA filler.

But there is an important difference between saying:

“The filler can often be dissolved”

and saying:

“Anything that goes wrong can be reversed.”

Those statements are not equivalent.

Once injected material enters a blood vessel and travels elsewhere in the body, the situation is no longer merely a question of whether a cosmetic result can be dissolved.

The fatal pulmonary embolism cases illustrate why that distinction matters.

Penis Enlargement Patients Should Compare Evidence, Not Marketing Words

Someone considering penis enlargement today may encounter:

• penis filler;
• penile filler;
• hyaluronic acid penis injections;
• autologous fat transfer;
• penis girth injections;
• penis enlargement surgery;
• penile implants; and
• other male enhancement procedures.

Choosing among them based solely on words such as natural, non-surgical or minimally invasive oversimplifies a complicated medical decision.

Instead, patients should ask:

1. What exactly is being placed in my body?

Ask for the exact product or device—not simply “filler.”

5. Has the procedure itself been clinically studied?

The Bottom Line: “Non-Surgical” Should Never Be Confused With “Safer”

The death of Igho Ubiribo following hyaluronic acid penile filler is tragic.

It also highlights a broader issue in the way cosmetic penis enlargement procedures are discussed.

A procedure does not become inherently safe because it can be described as non-surgical.

A substance does not become inherently safe because it is natural.

And an injectable product does not become FDA-approved for penis enlargement simply because FDA has approved that product for use somewhere else in the body.

Fatal pulmonary embolism following hyaluronic acid penile filler has now been documented. A separate fatal HA penile injection case exists in the medical literature. Fatal fat embolism following autologous fat penis enlargement has also been documented.

These are rare outcomes—but they are real.
